Clientele Hospital Cash Back Plan

My wife was admitted to ICU for an emergency operation on the artificial heart valve that was implanted 3 years back. She stayed in hospital for 8 days. I am now attempting to claim from my clientele policy and Clientele wants the following from me:

Claim form (to be completed by our regular doctor AND the treating physician. In this case it is a cardio surgeon whose time is extremely limited and very expensive)
Patient ID (understandable)
Medical reports (where on earth will a hospital give a patient the medical report)
Hospital account (this I can obtain from the medical aid hopefully)

I am extremely unhappy that Clientele is making claiming so complex and difficult. I now have to make an appointment with my doctor (R380.00) and the professor (R1600.00) for them to complete the claim form. The hospital will not provide the medical report so I cannot submit all the documents which will lead to my claim being repudiated for lack of sufficient documentation provided. I want cancellation and full refund of premiums paid. I phoned the hospital and was informed that under no circumstances do they give copies of files for to patients.
