HAVE NOT CLOSED ACCOUNT ON REQUEST

I paid up my Kulula credit card in full on 2/9/2013. On receiving a further statement from Kulula on 12/9/2013 I contacted the call centre and requested that the account be closed as I had already paid the account in full on 2/9/2013. Kele advised that the account would be closed and the R8.93 balance on the account will be reversed.
The reference number for the call is 4000 0926 6000.
I now once again receive a statement dated 11/11/2013 with a balance of R9.06. The account has not been closed and the balance was not reversed.
I want this account closed and the balance reversed!!! I will not pay for any further services or charges on this account as I have paid it up in full. I expect written confirmation of the closure and that it is paid in full.
