Payment for deceade spouse

Good day please not that I have opened a family cover for my family years ago and covered my family. my spouse pass on the 11-09-2013and I submitted the requested documents to Old mutual. when I do follow up I was told that my claim wont be paid nor refunded because the deceased was insures twice. my question was if he was insured twice by two different people why they didn't inform me because I think I insured him long before the second person that they claim s/he insured him. then I requested them to refund me my money that was paid for the past years then they inform me that I cant even get that money. so I need an explanation why I was not paid while I have been paying for so many years. I have done affidavit that they requested and a proof from maintenance court that he is the father of my child and was paying the monthly installment there. when I open the policy I was not questioned my integrity now that I claim I am asked all this integrin question. i need to know where is it writing on you policy that if a person was insured you decide, whom you want to pay. lastly I want to be paid my claim or refunded the money that I contributed, other policies didn't give me this probm.
