Three calls and still no tax certificate

We contacted the call centre to request a tax certificate for the tax year 2012/2013. We were informed that the certificate would be faxed immediately. No certificate. The following day we called again, another promise - again no certificate. The following day we called again and arranged that the certificate would be faxed and that the call centre would call us to confirm that we received it. No fax - no call. What do we have to do to get a tax certificate faxed to us? Reference number 65267084. The call centre is a bad reflection on the bank and dealing with someone with no accountability is frustrating - can a person with some accountability please follow up on this.
