STANDARD BANK HANDING OVER WHEN ACCOUNT PAID UP

I got a call from Standard Banks Attorneys MacRobert informing me that I have been handed over as my account is in arrears. I have been on debt review paying up this loan from October 2009 - not ever missing a payment. According to my records and my debt counsellor a spreadsheet was given to STD Bank in 2009 showing payment plan. The loan was paid up in May 2013, the last payment made in June 2013.
I contacted Standard Bank requesting a statement of my Revolving Credit Plan Loan from October 2009 - June 2013, I was informed that the request can be done but that I would have to pay R20 per page? I must pay for an error on their side!!

I informed the bank that they have handed me over when my account is paid up and I need proof of this, Mavis advised that the Debt Review Counsellor needs to phone and request the statement for there to be no charge. She gave me a contact number which I gave to my debt counsellor and he was informed that they cannot do this and that I must phone and request this? What is going on there!!

This needs to be investigated and sorted out, I have paid up this account but no one can help me with a statement and if so I need to pay R20 per page.
