Policy refuse to do payments

Good day. My mom Mrs SA Botha id number 530904 0091 084 passed away on the 31 August 2013. She did have a hospital plan policy and she was hospitalizing from 9 August 2013 until 30 August 2013. Jo ann from Sanlam contact clientele and she did send all the information from the dr site. Clientele told her that they will contact Mrs SA Botha family directly since today they have not been contact us. So I did phone them and ask what's the problem and they said they will send us forms again to be full in but we haven't receive anything. And we did send them all the forms. Claim number 404289389 not to be claim and we wonder why. They also busy working on claim no 610291837 for the last few months since my mom passed away ref S0024969 . They are very unprofessional why we have to wait so long for clientele to do payments. And keep faxing those documents but no payment yet
