Unpaid Item Charge

I have activated a 'stop payment' for one of my debit orders a few months back which is suppose to be valid for 6 months but still this debit order goes off my account and I get charged an unpaid item charge of R138 for a couple of months now already. Please refund my hard earned cash before i decide that we should part ways, that would go for my business account as well.
