Uknown online debit in my account

I had reported an unkown debit in my account on the 27 November 2012 to F.N.B, which reads BIGFISHGAMES 1635528. firstly i phoned customer service was told its a problem of which i reported it to as i was told. I asked for my card to be cancelled so that i can get a new. I was then told i needed to visit the branch of which i did. Went to Eastgate branch spoke to a customer service consulted, went with all my particulars ID, the incontact message, printed the bill out. When i got there it showed that the transaction went off my account really to me the consultant was unsure of whats happening with this online transaction, i was then told the problem has been sorted the money was credited to my account, went collect a new card.
The consultants words were, i did not have to fill the affidavit because money was creditted back to me.
After few days i was getting messages from F.N.B to submit an affidavit this confused me totally because when you visit the bank and a task was done to the account there is usually a report that has to be given by that consultant. Now that wrong debit order debitted my account again now what must i do to fix this? I have a new card but it went off.
