Telephonic Query since 2010 not resolved

My wife was involved in a serious accident on 13/8/2010 - Policy was sold to me telephonically - approximately 5 to 10 yrs ago - did not receive documents or contract - subscription premiums deducted monthly vias my salary advice - Contacted AIG with policy no - said that I did not qualify for claim as accident does not cover disabilitis as patients will recover only upon death benefit will be paid - I diputed and was put on hold - transferred to various departments only to be automatically cut off. tried on numerous occasionsa and was told that the claims documents will be posted to me. To date did not receive any documents. I Called again this year in March April and was told that I should have lodged compalint and claimed within 3 months - How do I do that when I was not given any documents. Can I still lodge complaint/claim - My wife had to be medically boarded from employment as she suffered more than 76% disability and was unable to continue working.
