Two weeks ago I called Liberty Life call centre to get assistance in cancelling the policy I have with then, I spoke to a gentleman, who told me that I need to complete a policy surrender from and attached my copy of ID, Proof of Residence and Bank Statement and forward the form back to them. He then promised me to forward the form to me within 10 minutes but till today I have not received the form, so I decided to call again yesterday and I spoke to a lady who also promised me to forward the form to me within 10 minutes to, as I am writing this complaint I have not yet received the said form.
All I need is to cancel the this policy, so I need the form please
