Funeral policy and education cove

I have a funeral cover and educational with old mutual since 2002 and i have been paying my policy fees every month. I changed my bank account from one bank to the other in January as my other bank was giving me grief, i have been paying - R800.00 month of which the policy cover for my family, my extended family and for my child's education plan. i was called sometime last year that my policy was in arrears and i made arrangements that they should deduct whatever amount that is left behind i=on the 15th Dec 2012. i thought that all is well but to my surprise i received a call from a lady by the name of Brenda informing me that my policies has lapsed how i do not know and she referred me to one of the branch so i can make arrangements to pay the outstanding amount of which i do not know is how much is behind. My question is how possible is that the policy can lapse without being informed and again if there was any issues with my policy why weren't i contacted telephonic-ally or via email as i received an email informing that my premiums are going up and so forth. i send several emails to your info department but nothing has happened except for the call that i received from Brenda
