Deletion of my debit orders without my concern

I have three policies with Old Mutual educational plan, retirement annuity and a funeral policy. In April 2013 my debit order bounced which was my fault, I was planning to pay the debit order that bounced manually. However a lady from old mutual contacted me and advised me that I could just add an extra debit order each month for the debit order that bounced. The arrangement was for my normal debit order to go off plus the educational plan arrears, then the normal premiums plus the funeral cover arrears amount then the normal instalment plus the retirement annuity arrears amount. However that lady deleted my normal debit orders and loaded only the arrears amount. Now I am in arrears with about 3 months of premiums because of this mess she did on my account. I have to pay arrears amount which I did not create I would like this matter to be resolved because it will cause my policies to lapse and this was not my doing. I would like the recording of the call I received in April 2013 to be retrieved because this is money I don't have that is going to have to come out from my pocket.
