Strugling to change detail on policy from 2009!

Old mutual has linked the policies of my previous nanny's funeral plan (which I paid) with the education policy of my son. We have been trying since 2009 to delink the policies and change the debit order for the nanny's policy. In 2011 I re-married and since then; we are also trying to change my surname on my son's policy. In 2012 I was phoned by their head-office and apolised too; (with letters sent to me) and promised that all my details has been updated and corrected (and I assumed; my new brokers listed)
Then last week I received a letter -
1) The funeral policy was still linked and the d/order never changed (I have paid in excess of R2500 towards a policy that is not mine / had no intention of paying anymore) I cannot stop the d/order as it is linked with the policy I actually would like to keep.
2) My surname has not been changed
3) When my brokers tried to sort it - they were told that they are not the listed brokers.

Dear Old Mutual - this is taking 4 YEARS!! You are USELESS. I will never recommend your products and I will tell everyone I know how your service is. It makes me stress now about what the service will be when the policies must pay out???
