Credit Check still reflecting payments

I cancelled my ADSL and Telkom line a long time ago. I recently applied for a home loan to find out that my Credit Check shows that I am still paying them roughly R600/month... why is this on my credit check when I have cancelled a very long time ago!!!

This could cost me my bond application!
