Upgraded to iPhone but no SIM as promised

This actually two complains in one:-

1) I upgraded my phone last week (telephonically) and was very happy to receive my iPhone 5S very soon after. I was quite impressed as it was processed very quickly and the service was friendly, a great experience. Then I opened the box and realized that the new SIM card to fit the phone was not included in the box as discussed (even though I am to be charged for it). Well no problem right, phoned the number given to me originally (should I wish to track progress or have any problems), quote U reference number, the call centre agent it was her department now and transferrred me. Great, the next person apologised and then said I will receive my SIM shortly and not to go into a Vodashop as I had offered. Great, except no SIM yet and no progress. Tried to call centre again and no one knows where the SIM is. I wasn't given a new reference number. I've been transferred to three numbers and no idea what to do. All three say it's not their problem. What do I do? I feel I'm in a void of note!

2) Second problem is the lack of an ability to communicate with Vodacom. But once in a void its a nightmare! No email address on website (link doesn't work).
