Failing to cancel contract, not keeping promise to

We have been trying to change cellphone contract after 24 month period to pay as you go contract. In August when contract expired we have phone the 808 number and was told the change would be registered in 24 Hours time. Nothing happened and debit order went through again.
9 November 2013 we went to MTN, Cape Gate, Brackenfell and spoke to Anel. We where informed that a different form needed to be filled in now, which Anel completed and put into the MTN system. Anel promised to phone us the next morning. We tried to prevent deductions made again. Promise where not kept.
Debit order went of again on 16 November 2013. We returned to MTN Cape Gate Brackenfell and where told by Anel that she completed the incorrect form and would recitfy it. As customer, we just had to kept on paying since August 2013 while being misled by MTN the whole time.
We where told that we would not be refunded for MTN failing to change old contract to pay as you go contract and we are insisting a refund.
