Phone fixed twice but still experincing problems

I had a new Nokia Lumia 520 wich was purchased on the 18th August 2013 and started to experince problems, it was then sent to Vodacare for repairs on the 2nd of September 2013 and came back on the 19 september 2013 with same problems!!! And it was sent back AGAIN mid september, I received it on the 13th of October 2013... now it is even worse because it is not charging at all. My worry is I have suffered enough, that phone is still new and has been going in and out of the repairs and when it comes back it does not come with the report that explains the diagnosis and the steps taken!!!

The last time I checked with Cynthia at Vodacare she informed me that they won't issue/replace it until Nokia gives them a go ahead, it was sent to Nokia and still comes back not working... I have suffered enough by returning a new phone several times and still comes back un-repaired.
So if it is my duty to request for a new phone from Nokia then I am making a Plea to Nokia to replace my phone...
