Did an Upgrade and not one thing i asked was done

I went 2 Vodacom 4 U @ Nicolway Bryanston on 11/12/2012 & renewed my contract, took a Galaxy Note 2. Whilst doing the upgrade, i asked Greg twice 2 check for sim insurance as he wanted 2 charge me 4 the Sim Swap as Note 2 takes a Micro Sim. I also requested that he remove a current data bundle that i had on my contract as i no longer required it. Was this done? No! When i recvieved my Jan Invoice i saw i had been charged for another data bundle. I then went back to the store and dealt with a guy Bert who was helpful. He checked & saw i had sim ins & refunded me 4 the Sim Swap Greg had charged me 4 that i enquired 3 times. Bert who assisted me with the refund then took my details & a screen dump of the active data bundle & said he would pass this on 2 Greg 2 sort out as he needs 2 refund me not the store. It has been over 2 weeks & i am still waiting. I visited the store today & he saw me, i left as it was busy & i was not waiting any more for his complacency. My phone has now gone faulty and they wont swap it out under the 6 month CPA Rule. I pay an add monthly fee for it. The trimming is coming off and battery 2, 3 hour min use! How does this not qualify for 6 Month CPA Swap Out?
