Telkom W/Ender Plan added to my invoice?

I'd like to know when the Telkom W/Ender Plan was added to my account - when opening the account I specifically stated it would mainly be used for ADSL, I did not request options such as this because its not something I require as we hardly use the phone.

The only reason I picked it up is because of the billing mistake in March.

If someone could please get back to me when this service was added, and under who's authority - I'd also like it removed immediately. I'm paying R167/pm for something I don't need.
