CCover's poor service

My Nokia N9 is insured with CCover. On 27 April 2013 it fell and the glass was damaged. Since it is insured I went to a Cellc store, Carlton Center in Johannesburg. I completed a claim form (Incident Form) & repair forms. Cellc store promised to dispatch my fone to Cellc repair center together with the forms I completed. Cellc never contacted me. On 14 April I was contacted by Cellc repair center. I was given a quote of R1700.00 for repairs. I told them to rather return my phone. I contacted Cellc about the insurance. I was referred to CCover. At CCover I spoke to a lady called Natasha. She told me my forms were not submitted. She asked me to fax or email them. I faxed. On 21 May I contacted Ccover regarding the forms. I was told that they were not sent. This is despite the fact that I sent them via fax on 14 May. I then emailed the forms on 21 May. I contacted CCover on 29 May and spoke to Virginia. She said they were not sent. This is despite the fact that I sent the forms to CCover twice. Virginia located the forms and promised to contact me before close of business today on 29 May 2013. She did not contact me at all. Fone has been returned without being repaired. What to do?
