No Insurance thanks to incompetent staff!!!

On the 1st of May'13 I took my phone to Mtn, Soneike (Igshaan Davids) who advised me my screen was cracked and that I should contact my insurance (Call Sure) I contacted them and was shocked to hear that my phone was never insured even though I have a copy of the signed form which Igshaan and myself signed (June 2012) He advised me at that time he would send off all the relevant docs which he surely never done else I would not be complaining!!! Now I am stuck with NO insurance thanks to Igshaan Davids. He contacted me a few days ago saying that I should pay R704 R500 excess for a repair of R300. I find this very pathetic as I would not be in this situation had Igshaan sent the form off in June 2012. No he feeds me all kinds of stories about they were using an old system back then etc, etc . bottom line, he messed up and I want answers!!! Who in this day and age still gets monthly bank statements posted to them to check if a R64 has been debited off your acc. you don't even get cell phone notifications for such small amounts...
Kindly advise as to what actions will be taken in this matter?
