Poor customer Service

I was using vitality and I've been paying R85 a month. Early this year I left witbank to go stay in pretoria. I called the gym for membership cancellation and they told me to come to the gym offices. I did as requested and filled in a cancellation form number 7191 from the cancellation booklet at witbank Planet Fitness on the 24th of May 2013. They told me the system renewed my contract automatically, and due to one month waiting period the debit order will go through untill June 2013.As I speak the gym is debiting R355, the debit order has not been stopped and the account is not cancelled. I request that the account be cancelled and the debit order be stoped since it is damaging my credit profile. I've been calling for almost 20 times now but nothing has been dome regarding this case. I'll have to take legal steps if this is not resolved urgently.
