Hopital Cash Back Policy Document

On 05/02/13, I phoned edgars financial services taking a hospital cash back. I spoke to Nthabiseng. I haven't received the policy document to check whether the information is correct or no. On Saturday 30/03/13, I received a statement with premium deductions R113.00 and once off fee R76.50 of which the consultant didn't mentioned to me. How can I pay premium without documents. 02/04/13 at 08:49, I phoned the customer service spoke to Lethiwe Miya asked her to cancel this hospital cash back.

I want this R113.00 premium and this R76.50 (R189.50) to be deducted from my account since I don't have anything that involved me with it. I am no longer interested in this policy. If they fail to provide me with a simple document, how possible it is that they will pay me when I am admitted. I lost trust in them. I am hoping that this issue will be sorted out as soon as possible.
