Dispute on fradulent use of my account

19 October 2013 i went to Ackermans Sammy marks, to pay off my account i owed about R300, when i got there i realised that my balance has increased it was about R3400, i blocked my card immediately, because it was missing. Tebogo asked me to bring copy of ID, dispute form and affidavit from the police station on Monday, so that she can fax the documents for me and i did that (20 Oct 2013). i have not received any feedback since then. i went back to Ackermans Sammy Marks (on 19 November 2013 and I was told Tebogo does not work there anymore. today 20 November 2013, i called the call centre and they told me that they have never received my dispute documents. i faxed the documents today (20 Nov 2013) and i called call centre again to confirm receipt of the documents, and i was told they did not receive the documents. what must i do to get assistance from Ackermans, my card was used at Ackermans Bloed Street and no one is prepared to assist me. i have all the documents they asked from me.
