Being Registered for Services without my consent

I would like proof of where i registered for OTA. I see it can only be done by one of the following ways.
How to register

Register in-store at any till point with your Edgars store card and Green ID book
or call the Call Centre on 0861 788 211

I never performed any of these options.
I want this removed immediately and if there are any costs it is for your account.
Failure to remove me will result in me obtaining legal advise on this matter.

PS I will also be closing my account because of this.
