Why request info when same is not used when sent??

I am writing this report on behalf of my 78 year old grandmother, who ears a mere R 4, 100.00 per month as pension. She is a member of Sizwe Medical Fund and received a notice on 8 November 2013 to send her proof of income in order to apply the correct bracket for payments. Please note that she paid - R 1, 900.00 per month until Dec 2013.

I personally e-mailed the proof of income to [Email Removed] on 25/11/2013 @ 01:42 PM.

Then, on 2 Jan 2014 an excessive amount of R 3, 080.00 was deducted from her account (which was definitely not the 8.9% increase promised in the letter dated 18/11/2013).

Upon inquiry on 6/1/14 I was advised that you NEVER received this mail. I then faxed AND e-mailed this document AGAIN. I followed this up EVERY day only to once again hear today that you still didn't get it and I re-faxed it AGAIN - now this time to Claims Department as Membership has a "back-log".

I need a COMPETENT person who can attend to the update of your records and the refund to be done to my grandmother as she is left with a shameless R 1, 000.00 for Jan 2014 due to your deduction, which in my opinion is due to your incompetency.
