Claim not cover by hospital benefit

If the doctor requires a scope while you are in surgery Discovery refuse to pay under the hospital benefit (although my option allows for 200% benefit). If the same procedure is done in the practitioners room they are willing to pay the full amount out of hospital benefit, how ridicilous. Always try to get more from the customer.
