No Change on my debit

My contract ends the end of January 2013. But I got another phone via the insurance. From a blackberry to a Nokia in October, So I don't need the BIS and I also cancelled the Insurance because there were major issues regarding what phone they wanted to give me. The contract is R200 per month but with all the extras it comes to R425.48. I have cancelled all these extras in November Last year all ready and still they deduct the full amount. They guaranteed me it will be off. The Cell Phone Number is question is 078 801 4522. This is obsolete poor service from Nashua in 3 Rivers and not to mention the branch in Vanderbijl Park.
