Extending loan period to reduce payments

I contacted FNB Collections department about extending my loan repayments over a longer period to reduce my installments as I have had medical expenses and cannot meet all my financial obligations. They said no, would not even consider it. How can FNB help you? They cannot! So my next option is debt review. Surely they would want to prevent this!!!
