BILLING ERROR

To whom it may concern,

I joined this insurance in December 2012, and they only sent forms to be completed, no insurance card was sent, however they take money from my account every month. (account 197417999606001718)

How will the hospital know that i am covered by this life insurance, if i do not even possess a card. this is very unprofessional.
