Change of my account with out my authorisation

Went into the bank today to draw a large sum out of my account, when i got to the Teller the could not do the payment because of an error. The lady spoke to her manager about the problem, who said its probably because my account has been changed. Why was i never notified about the change or asked if i would like to change my account??? I did an EFT yesterday the money went out but the was never received, is it because of my account been changed??? Will that payment get paid now or have i now lost my hard earned money???
