Not authorised debit order cancellation

This letter refer to policy number: H566336554
I had a debit order with African Life relating to the above mentioned policy. According to African Life Insurance they received an instruction from Absa to terminate the debit order around May 2009.
I made an enquiry with Absa and they seem not to know anything relating to termination of the debit order. To my surprise I have never issue an instruction to Absa or African Life to terminate the above policy. I don't understand why the policy was terminated because:
o I still use the same bank account.
o There is always availability of funds.
o I did not authorize termination of the above policy.

My concern is this matter came to my attention this morning when I went to your offices to lodge a claim. To my knowledge I thought everything was in order as I did not authorize the termination of this debit order.
It was not necessary for me to check monthly bank statement because I knew that this debit order was still running.
