I reported on 25 October that I required a copy of the life insurance claim forms to the client services department in writing. I have also requested claim forms! I have breast cancer which is a dreaded disease under which the life cover will cover this account.
I keep getting that they need personal medical information but not that I submit a claim form. I need a copy of the life insurance as well as the claim form before I get lawyers involved please.
My oncologist will not divulge personal information without an official claim form. I have asked for this about 10 times since and had same response!
