Unable to reniew my credit card

In March 2013 I received a sms to fica my credit card account, which I did at Mall@Carnival branch. In April my card expired. At the end of April I went to the same branch and supplied updated documents again, of which they took copies and iformed me that I will receive an sms within 5 days to collect my new card. A week later I went into the branch again and was told they are still waiting for Head Office. Never heard anything on the 19th of June I went to the Springs branch who informed me that it is still with Head Office. I requested them to follow up and was told you will receive my card soon. On the 15th of July I called "Head Office" on 0861 000 555 and was told I have to FICA my account and was told to go to the branch again. Since then I have sent numerous mails made lots of calls and still nothing. I have mailed my details to [Email Removed] again on 29.07.2013 on 08.08.2013 I received and sms to inform me to fica my account. I called this morning and was told to go to the branch, then I receive an sms saying my card has been activated, phoned again was told to go to the branch. Almost 4 months since my card expired and being charged a monthly - no card
