Accidental Plan opened without my knowledge

I would like to have this matter investigated as to how an Accidental Plan policy was opened without my knowledge by BLUEBEAN, the plan has been going on since 2010 and they never sent me any policy schedule or even agreed with me regarding the beneficiaries instead they went ahead and debited my account without my approval.

Details of the policy are:

BLUEBEAN Accidental Protection Plan: MDR001424011

Inception date: 10-08-2010 (without my knowledge)

I want all my premiums paid back to me in full as my bank account was debited by BLUEBEAN.
