Payment stopped to wrong account

I have been on Debt Review since 2009 and between 2 companies since then. In 2010 NCA moved my file over to Consumer Assist and also thought they were on the ball and they might help me, but i was wrong.
An account was paid in full and asked the counselor to please close the account and move the payment towards another account. He confirmed and said it was done. In November 2012 i asked the bank for a balance and to my shock the account has never been paid. Upon closer investigation i found that the counselor stopped payment to the wrong account and is now back to square one. i then received an email telling me that they will not be held responsible for that mishap and me as the consumer is now again held responsible for his slip up.
A company supposed to help me with my debt, made a slip up and pinning it on me now. I want the matter to be taken seriously as i have consulted a lawyer for this matter and will be advised.
