Negative smart device account, NO device!

I ordered a Ipad November 2012, but FNB could not keep their word on the delivery date, so it was cancelled and I bought a Ipad cash from Apple.
3 Months later, device insurance was deducted from my account. After a few weeks of battles, I was reimbursed recently.
Now 6 months later, FNB creates an Smart device account on my name with a balance of -R4070.

What is going on? Please rectify this ASAP.
