Credit Life Not used, client held responsible

I had a personal loan with Direct Axis and i purchased credit life with this loan. At the time of debt the company i worked for went under liquidation. I faxed details of the liquidation letter with my loan details to direct axis the Monday after the company closed hoping the credit life would kick in. My husband and i worked for the same company and we both had no jobs for a couple of months. I failed to follow up on the credit life plan and assumed the account was closed. I finally managed to finish all my debt by going under debt review and last week Friday for the first time in 3 years i was told by my debt counsellor that i cannot get my clearance letter as they picked up something from Direct Axis. The funny thing is that it was not listed on ITC or i would have known and addressed it long ago. I regularly requested credit reports to make sure i finish all my debt as planned with debt counsellor. I am in shock as i am now accountable for R20 000 and what happened to the credit life plan? After 4 years of struggling to finish all my debt i finally finish and then to learn i owe R20 000 to direct axis. This is so disgusting! I am unsure of what to do.
