Non existent data service

My data package worked fine but the last three weeks it is non existent. I can connect but then nothing happens. I phoned the service provider who told me they are working on the problem. One week later I phoned again and was told the same story. I then sent an e mail to customer care who informed me today (six days later) that they apologize and attached is a form I must complete. No form was attached. I feel I now pay for a service I cannot use. In short I'm.
