Staff refusing to help and forward my documents

A contract was issued using my details, i only noticed when I got sms notification on my mobile from my bank. I then phoned nashua mobile and they told me that it was a cell phone contract opened at Nashua Mobile Galleria in Amananzitoti Durban. I took a drive to the branch and was assisted by lady called Thobile/Thobeka and she said I need to make an affidavit stating that I did not take out a contract and a copy of my id book bring it back into their store. I did this but the following month the debit order went off again. I then went back to the branch at Galleria did not find the lady who helped me but an indian lady who refused to me her name and told me that she knows nothing about my claim, she then sent a text message to the store manager (who by the way is never there) and the store manager said she had sent my details through to risk department and will phone the next day when she arrived with the case tracking number. days went past and I recieved no phone call or text message with tracking number. A another debit order went off again, I then went back to the store and recievd TERRIBLE services. I phoned the risk department and was told nothing was ever sent.
