Cancellation of contract

I cancelled my contract on the 11th of October 2013 via e-mail as per the instruction by one of their call centre agents. On the 25th of November 2013 I called them again to find out if it was cancelled, I was told that I should have completed a form. A form was e-mailed to me and I completed it and mailed it back to them. I've been phoning them ever since I sent the request. They always promise to come back to me but they never do. I'm really tired of chasing after them now. Three monhs later I haven't heard anything from them. Contract still not cancelled, I dont know what to do now.
