Debited several times in one month

Premier Advantage Card have been debiting my bank account several times since November last year (2013). I have phoned them and I have also sent them e-mail and fax. I haven't had feedback from them instead they keep on debiting my bank account. They sent me their card in October 2012 which I activated over the phone then cancelled it within a week, I never used their card. To my surprise from November last year (2013) they started debiting my bank account with different amounts i.e. R300, R295, R290 and R199. When you phone them they keep you holding.
