Contract opened with my personal info

On the 28.03.13 I received a bank notification of R826.50 that went off for Vodacom. As soon as I was made aware that someone frauded with my personal details I immediately contacted Vodacom informing them about it and to find out what I needed to do. I forwarded all the documents through but nobody contacted me. I ended up calling them numerous time and speaking to different people. The last person I spoke to was in credit and he told me documents wasn't received, he also added investigation takes about 2weeks. Apparently I took out a second contract with them on the 24.02.13. In April they debited me yet again. I have never taken the contract and never even received a phone. Not once did any consultant try to contact me all I've been getting was letters of demands. I called them yet again and requested the call that was made for the phone; I was told to hold the whole time. Due to this my credit record has been badly affected and now I can't get credit anywhere for something I clearly never done and who is suffering now due to Vodacom's lack of service and unprofessional consultants; my children.
