Policy not activated

On the 16th January 2013 I went into the Vodacom to ensure my mobile phone. R149 was meant to be debited off my account monthly as the premiums. The first debit went off and thereafter I didnt notice. Eventually upon realisation that the debit is no longer running I queried this with the call centre and they advised me that I did not have an active policy. I went into the Vodacom Shop in Fourways and the gentleman call the administration office and they were able to view some history. Upon going back to Vodaworld, they advised me that they cant see anything once again and they are not able to assist me. As a result my mobile phone is not ensured and this is not a loss to Vodacom as I still continue to use their network.

I would like vodacom to review the footage of the day I applied for my insurance for my mobile as they now claim I never activated the policy. When the activation is not in my control.
