Debiting account without authorisation

In July 2012 an amount of R29.95 was deducted from my account. In August 2012 they deducted twice in the month first R249.95 on 24 August and then R99.00 on the 31 of August. I contacted the bank and they told me that it was for life insurance. I don't have any insurance with Platinum Life, so I cancelled it at the bank. I tried to contact the call centre to sort te matter out: however they persons are not helpfull at all, and very rude. They had my details as an male by the name of Morris Dlamini, but I am Female and my name is Annetjie Henning. We tried to get through to Elma Malan, but she is never available for assistance.
When we phoned early in January 2013, we were told that the transfer was done and the money should be in my account. I check and there was no transfer done. I phoned back and told them that there was nothing in the account, I asked them for a ref. no. and dated, they could not give it to me. I asked them to send the proof of payment and that is also not done yet. I phoned again on the 21st of January 2013, and still no money; however they insist that the transfer was done from their side.
