Cancellation not Processed

I cancelled my gym membership last month, but was still debited for it this month.

I even received confirmation in writing that it was cancelled, however someone dropped the ball along the way.

I want this cancelled once and for all, and I want a refund of payment which was added to my Discovery Health policy amount.
