Transfer of points to new card

Our Smart shopper card was lost and eventually did we manage to get hold of someone who could tell us what to do. We completed the necessary document to have the point transferred from the old to the new card.
This was sent to [Email Removed] as per document.
We sent this mail on the 18th of December 2012 and today more than 3 months later those points have still not been transferred.
Can this be done or should I ask does the staff know how to do this??
