POOR CUSTOMER SERVICE

I went to above store and inquired about an application form for a lost smart shopper card which they had me fill in about a month ago and were supposed to fax through to lost cards. Last week I called the Customer Care line and asked if they could check why my points have not been switched they said they have not received my application and I must go back to the shop and get the application. When I entered the store and explained what I wanted was shuffled from one person to another and had to repeat myself over and over again each time. Eventually I was referred to the supervisor whose attitude was atrocious, she acted as if she does not want to assist and ignored me, she was then told to check their file and still ignored the instructions, I ended up leaving without being helped. All i want is for them to send my form to the relevant department or even just to me so I can fax it myself. Thanks
