I had reported a plasma being to you but still no response, my problem is it was when my spouse was there so it's him who wrote the affidavit. I work at Mafikeng and I reported this matter to Ellerines in Mafikeng and they said they need the affidavit not the case number only. Should I give them that affidavit that my spouse wrote and will it be possible for them to fax me the insurance claim so that I can sign and fax it back to them together with the affidavit because I can't go to heidleberg.
