Cancelled Netstar being deducted every month

I cancelled my netstar in approximately June this year with the Rustenburg Netstar branch in person. Unfortunately I never checked if the amount was still deducted afterwards. Now I see that it is still being taken every month. The lady at the Rustenburg branch now informs me that it was never cancelled. She now wants the documentation from me. Surely they should have record somewhere. She blames the mishap on a temp that was helping out. This should not be my problem. As I pointed out, The client has no knowledge whether the assistant behind the counter is competent. The company employing her/him should ensure that their employees are. My perception was that the lady I spoke with now was losing patience with me and tried her best to make this my problem.
I would like this contract terminated from the date I filled out the docs. And my money paid back
